Lack of care coordination is among the top three sources of waste. I've noticed how little care coordination there is. Doctors don't refer to a patient's medical history or ask other medical providers what has already been done. So much time and energy could be saved if one doctor trusted other doctors' findings--and were willing to take the time to find out what is already known about a patient.
