A year or so ago I requested my medical records from the neurologist's office because the neurologist was leaving that office and I was transferring to a new doctor. I was dismayed when I found some serious errors in the record I received.  Voice-recognition software had been used--and apparently no one had reviewed the record for accuracy.


This is a known problem, it seems--according to NEJM Journal Watch, July 14, 2018--"Errors Common When Speech Recognition Software Is Used To Document Patient Visits":
