IDSA Issues Vaccination Guidelines for Immunocompromised Patients

The Infectious Diseases Society of America's guidelines for vaccinating immunocompromised patients and their household contacts are available in Clinical Infectious Diseases.

The society's recommendations include the following:

Specialists share responsibility with primary care clinicians for ensuring that patients receive the appropriate vaccinations.

Vaccines should be administered before planned immunosuppression — live vaccines at least 4 weeks beforehand, and at least 2 weeks for inactivated vaccines.

Household members should receive inactivated flu vaccine, and live attenuated influenza vaccines may be given in limited circumstances; they should not get oral polio vaccine, however.

HIV-infected patients should get vaccines according to the many age restrictions detailed in the CDC annual schedule — for instance, H. influenzae type b conjugate vaccine is not recommended for adults with HIV.
